State income tax withholding
Withholding account (Form BB-1, Basic Business Application)
Hawaii Department of Taxation
- Typical wait:
- About 3 to 4 weeks by letter (same day if you apply in person at a district office)
- You'll receive:
- A Hawaii withholding tax ID, written WH-XXX-XXX-XXXX-01, mailed to you in a letter. There's no fee to register.
- Send Unify:
- Your Hawaii withholding tax ID.
State unemployment insurance
Unemployment insurance account (Form UC-1)
Hawaii Department of Labor and Industrial Relations (DLIR)
- You'll receive:
- A DOL (unemployment) account number once DLIR confirms you have Hawaii payroll. New employers pay 2.4% plus a 0.1% Employment and Training assessment on the first $64,500 in 2026.
- Send Unify:
- Your DOL account number and your rate notice.
Register online. DLIR doesn't accept paper forms.

Temporary Disability Insurance (TDI)
Hawaii DLIR, Disability Compensation Division
- You'll receive:
- No state account. You buy TDI coverage from an authorized carrier (or use an approved self-insured plan). Employees can pay up to 0.5% of weekly wages, capped at $7.50 a week in 2026.
- Send Unify:
- Your policy information.
Other state program
Prepaid Health Care
Hawaii DLIR, Disability Compensation Division
- You'll receive:
- No state account. You provide a qualifying health plan to employees who work 20+ hours a week for 4 consecutive weeks and earn at least 86.67 times the Hawaii minimum wage a month. Their share is capped at the lesser of 50% of the premium or 1.5% of monthly wages.
- Send Unify:
